How the fuck do you get rid of blackheads and acne?

I have had this piece of shit acne motherfucker for 8 years now, I cant even remember the last time my skin was smooth.
I have been on so many medications but my doctor REFUSES to get me on accutane.
What the fuck can I do to get rid of it?
I am on antibiotics plus the strongest acne remover they had but its not doing shit.
Also how do I get rid of blackheads, whole face literally has black spots everywhere.
I also think the blackheads is making my face a bit bloated, the areas in my cheeks where i have blackheads are extra puffy

My diet is as clean as can be and has been for the past month, no difference at all in skin so far. Also drinking about 4L of water and no other fluid every day. No sugar, almost no carbs and no dairy. Doesnt help me at all unfortunatly

Laser blackhead removal, it also shrinks your pores. It's around 25$ a session, and you need to go a couple of times. I think it's pretty much permanent after that.
