In a lot of countries it is prescribed for premature ejaculation so just say and you'll get it. If for some reason it's not in your country just say you suffer from pain, after accident, or from sciatica etc. They might want to give you NSAID drugs instead but then just say you're already taking ketoprofen or something but your stomach hurts after it so taking it paired with tramadol helps you reduce the side effects. If they'd rather give you other opioids, say you're allergic or feel too dizzy after them. So say something like "Mr Doctor, my spine hurts, I can't take NSAIDs all time because my stomach hurts, but my mom/dad/aunt/grandma gave me these [generic tramadol name in your country] pills and they worked well, could you write these out for me? I only take them occasionally"
Seems like worst case scenario it might take a couple of weeks to be finally prescribed tramadol and multiple doctor visits. I don't know about other countries but a doctor visit here is the equivalent of 50$.

Yes, I was using it for pain management after surgery for about 2 years, stopped like 5 years ago. So if you're interested I can tell all the details. There are side effects but they differ whether you take it occasionally or constantly, what doses you do etc so it depends what exactly you want to use it for and what performance you expect. I'd rather advise against using it for longer than 3 months if you want to use every day, but if you only take occasionally (eg for sex) then it's okay. Side effects might be feeling dizzy, lower inhib, more energy and motivation, but also being irate and slightly aggressive, but that's rather rare and usually only after prolonged use. One annoying side effect is you might have slight trouble pissing, which I suppose is also why it's so effective with delaying ejaculation, probably shared mechanism.

Yeah I wrote that, it's actually 37,5mg that those smallest instant release tablets have in my country, those are german Zaldiar tablets (with paracetamol). Also available are 50mg capsules and 50mg ER pills, I'd say 50mg is upper limit for instant release (IR pill or a capsule) unless you have tolerance, but in case of extended release it should be just fine, just remember in case of ER tablets you should take it 1 hour earlier because it takes longer to absorb it.
