My E2 is 25ng/dl, prolactin is 11ng/ml, TSH is 3 (basically subclinical hypothyroidism) and every other hormone is pretty much in the normal range. It's been 3 years since i got it and i think it's gotten bigger every year. Is the cause hypothyroidism? Low insulin sensitivity?

I'm opting for surgery right now, should i take a very small dose of anastrozole every other day to keep the gyno from reoccuring after the procedure?

You can have gynecomastia despite normal e2 levels, maybe your T/E2 ratio is off.
Insulin resistance and hypothyroidism can contribute to gynecomastia but aren't direct causes in all cases.
