Either incredibly self aware because she looked pretty much normal and good by most female standards of themselves but was able to tell she had a specific problem she could fix and sought to fix it.

Or incredibly narcy because she looked pretty much normal and good by most female standards of themselves but was unable to deal with not looking perfect and unrelentingly pressured doctors to improve her appearance until she luckily happened across a doctor who knew wtf they were doing and actually fixed her face.

They're just moving the bones that already exist into the optimal position and the aesthetic result is whatever they happen to end up with. It's not that black and white, but closer to that than it is to maxilofacial surgeon designing faces.
