Otoplasty is great if you have a weak antihelical fold but a lot of times people also have a prominent upper helix which means even with an otoplasty the top of their ears will stick out, like this case:

This guy would also benefit from getting an earlobe reduction.

That's why I'd recommend getting an otoplasty only if you have a weak antihelical fold, otherwise I'd advise to go with macrotia ear reduction surgery which removes a piece of the upper ear bring it closer to the head and making the ear smaller, seen here:
