The one roadblock to reaching this mental state right now is how much I lament the unfairness of it all. This whole thing has always been a catch-22

Obviously, the approaching is rarely (if never) made by girls. And when it's done it always seems that the girls are below in term of SMV.

The very least they can generally do is giving unclear IOI's by making themselves present in your eye sight and wait until you open them up with a conversation.

Other than that there really is no other way to make things happen. Cold-approaching can sometimes be exciting but everything is at the expense of her being interested and available, which is not quite often the case.
